What is this form for?

This form is for any organization that would like request their trip to be recognized as a Victory Life Church sponsored mission trip. If approved, any VL Church staff members would qualify to receive paid time-off as outlined in the VL Church Employee Handbook.

Upon submitting the application, it will be reviewed by Leadership to ensure the trip is in alignment with the overall church’s vision. This form will need to be submitted for each trip the organization wishes to be considered approved no later than December 31st, of the year prior to the trip dates
